New Patient Information/Forms
Welcome to Agapé Physical Therapy! We are excited to be able to serve you and care for your needs! If you choose to come to us for physical therapy services, you can call any one of our four locations that is most convenient for you. Upon calling, one of our receptionists will ask you for some information and will schedule an appointment for you. If you have any questions, you may call us or contact us through email through our contact us link.
In an attempt to simplify and speed up our process on a patient's first physical therapy session (the evaluation), we have conveniently posted four of our forms that will be necessary for your chart. You can select each form individually below and print them from your printer. Once you have printed these forms, please read through them thoroughly and fill them out prior to your first session. This will help to save time once you arrive at our location. If you can not or choose not to print these forms, they will be handed to you on the day of your first session to be filled out.
